PROGRAMS OVERVIEW

Family Preservation Program
 
The Family Preservation Program is designed to work with families who are at risk by connecting them with professionals and services that can help fulfill the needs of families in crisis. The goal is to identify and strengthen families who may be at risk of having their children removed from the home. The program operates in the Big Sandy region of Kentucky, which includes the counties of Floyd, Johnson, Magoffin, Martin and Pike.


 

IMPACT PLUS Case Management
 
Case management and clinical services are for Medicaid-eligible individuals under the age of 21 who have a diagnosed psychiatric disorder, a six-month history of emotional and behavioral problems and are at risk of being institutionalized.

Youth Support Center
 
The Youth Support Center is a community-based after-school Intensive Outpatient Program. This program uses various therapies and activities to prevent problems that result in the removal of teens from their homes. YSC also helps youth transition back into their homes and community following a stay in residential care, foster care or the juvenile justice system.
 
On a judicial level, the focus is to reduce the number of low-level offender youth in Kentucky's jails, lockups and secured detention facilities. Many of the youth attending the program are referred by the Laurel County court system, as well as the Department of Juvenile Justice, the Department of Community Based Services, the Laurel County school system and Sunrise's In-Home Coordinators. The average stay is 30 days for court-ordered youth and three to six months for all other referring agencies.
