IMPACT PLUS
PROGRAM OVERVIEW

The IMPACT Plus program of Sunrise Children's Services is designed to empower parents and guardians to manage their child's behavioral and health conditions on their own and avoid the risk of institutionalization or other out of home placement.

The program serves at-risk children and families through therapeutic and case management services in Daviess, Hancock, Henderson, McLean, Ohio, Union and Webster counties in western Kentucky and Anderson, Bourbon, Franklin, Harrison, Nicholas, Scott, and Woodford counties in central Kentucky.

Sunrise also serves communities in the Bluegrass Region including Lexington, Richmond, and Danville, and also the region around Somerset in south central Kentucky, through case management services.

To be eligible for participation in the IMPACT Plus program, children and adolescents ages 3-21 years old must have a six-month history of emotional and behavioral problems at home, in school, and in the community and have a current Kentucky medical card through Medicaid. They also must have unsuccessfully tried a less intensive therapy, such as a school counselor or a community mental health provider. Referrals to IMPACT Plus can be made anytime.

Once eligibility is determined, the child will be assigned a treatment team that includes a Sunrise service coordinator, a behavioral health therapist, and the childs primary caregiver or legal guardian. This team of experts identifies the issues the child faces and puts together a care plan of action. At Sunrise, we believe it is preferable for children to remain with their natural families if at all possible. Our team of highly experienced counselors and service coordinators work one-on-one with families to reduce the childs risk of hospitalization.

In addition to service coordination, weekly in-home and in-school counseling services are provided to help facilitate change as needed in those settings. Sunrise believes in a holistic approach and recognizes that other factors, including lack of nutrition or other basic necessities, may contribute to a childs overall well-being and behavior. Service coordinators also help the family find other services as needed to improve the childs quality of life.
